The Medical Social Worker will assist patients and their families by providing emotional support, resource navigation, and care coordination to enhance their well-being and recovery.

Responsibilities

  • Conduct psychosocial assessments to determine patients' emotional, social, and financial needs.
  • Develop and implement care plans to address patient concerns.
  • Provide counseling and crisis intervention for patients and families.
  • Advocate for patient rights and assist with healthcare decision-making.
  • Coordinate discharge planning and transition of care services.
  • Educate patients and families about community resources and support programs .
  • Maintain detailed case records and ensure compliance with healthcare regulations.
  • Collaborate with healthcare providers to ensure holistic patient care.

Qualifications

  • Education: Master's degree in Social Work (MSW) .
  • Licensure/Certification:
    • Licensed Clinical Social Worker (LCSW) or equivalent state licensure.
    • Additional certifications in healthcare social work preferred.
  • Experience:
    • 1+ years of experience in medical or healthcare social work.
  • Skills:
    • Strong advocacy and problem-solving abilities .
    • Excellent communication and counseling skills .
    • Knowledge of healthcare systems and patient rights .
  • Travel Readiness: Willingness to relocate temporarily for assignments.
